- Submit a practitioner application using the Application Tool
Submissions are processed in an average of 30 days. Allow time in advance of a provider’s actual start date for the application process.
Licensed hospital, facility or entity types interested in applying to participate in a Wellmark network must complete contracting prior to credentialing.

- Once contracting is completed, submit the Facility/Entity credentialing application in E-cred Central
