Coverage while you travel
With Wellmark’s Blue Medicare Advantage PPOSM plans, you can travel across the country with confidence, knowing you're covered if care is needed. You’ll have access to the expansive Blues national network and enjoy in-network rates when visiting participating doctors or hospitals. If you choose to see a provider outside the network, a higher cost-share may apply.
Heading abroad? All Medicare Advantage plans include emergency coverage worldwide.
Vision coverage
Your Medicare Advantage vision coverage includes a $0 copay for a routine eye exam once a year.
Plus, for a $0 copay, you get full coverage for single vision, lined bifocal, lined trifocal, lenticular or standard progressive lenses every year. You’ll also have an allowance for elective contact lenses or eyeglass frames every 12 months. You’ll pay any charges above the plan’s allowance.
Your vision benefit offers access to a large national network of providers, including chains like Walmart, Sam’s Club, Costco and Visionworks. — just present your health plan membership ID card at any location. Find an in-network provider: vsp.com/choiceretail.
You can use out-of-network providers, but you will be responsible for costs above the plan’s approved amounts. You will also pay 50 percent coinsurance for out-of-network services.
