The Live Healthy Blue prepaid Mastercard®
As you complete Live Healthy Blue healthy activities, the rewards you earn will be loaded to a Live Healthy Blue prepaid Mastercard. Shop using your Live Healthy Blue rewards card at participating local and national retailers.
You’ll receive your rewards card approximately two weeks after your application is successfully processed.
Cards will be delivered by mail with activation and program instructions to help you get started. Program registration is required to earn rewards. There will be $0 on the card until you start to earn and redeem rewards.
First time user?
- Go to WellmarkAdvantageHealthPlan.com/Login
- Log in or create an account to access your member portal.
- Select Manage Wellness & Rewards to enter the Live Healthy Blue site.

Log in to the member portal now and access Live Healthy Blue directly via your single sign-on.
How it works
- Log in to your member portal at WellmarkAdvantageHealthPlan.com/Login and click Manage Wellness & Rewards to go to your Live Healthy Blue account.
- Review and complete your list of eligible healthy activities between Jan. 1 and Dec. 31, 2026.
- Redeem your earned rewards, so they can be added to your Live Healthy Blue rewards card. Redeemed rewards will be loaded to your rewards card within 48 hours.
- You can find a list of participating local and national retailers in the Live Healthy Blue portal or by calling 877-271-1467 (TTY: 711).
