HMO

A good option if your employees live in Iowa.

Plans available in the Wellmark Blue HMOSM Network give your employees access to more than 32,000 health care providers across all of Iowa’s 99 counties and in bordering counties. Some HMO plans may have a variation in benefit costs when employees establish and visit a designated primary care provider (PCP).

PPO

A good option if your employees need national flexibility.

Consider the Wellmark Blue PPOSM Network if your employees travel frequently for business or pleasure and are more likely to need medical care out-of-state. With an extensive statewide network and a national network of providers through the BlueCard® PPO program, your employees have access to more than two million unique, in-network providers throughout the U.S. at a premium price.

POS

A good option if your employees live in Iowa and want out-of-state coverage.

The Wellmark Blue POSSM Network provides a flexible network option for employers and members who prefer the lower cost of the state-based HMO network but would like protection from higher out-of-network costs. This provides the pricing relief and in-state coverage of an HMO, but gives greater flexibility in case of unforeseen, complex conditions.

Choose a plan type

From traditional coverage to high-deductible health plans (HDHPs), we offer a range of solutions to help meet your needs.

Modified plans

Similar to copayment plans but eliminate coinsurance entirely.

With these plans, your employees will have lower copays for visits with their primary care provider (PCP) and no coinsurance. Visits with non-primary care providers will have higher copays.

Copayment plans

Provide predictable copays for many common health care expenses like in-network office visits.

With these plans, your employees will have varying copays for covered office services depending on the provider, while other services are subject to deductible and coinsurance.

Primary plans

Offers a split office copay in a traditional plan structure to incentivize members to seek care through their PCP first.

Like modified plans, your employees will have lower copays for visits with their PCP and higher copays for non-PCP visits. With these plans, however, most other services are subject to deductible and coinsurance.

High-deductible health plans

Encourages members to engage in their health plan by paying the full cost of care until their deductible has been met.

These plans take care of covered health care expenses once their deductible has been exceeded. These plans also qualify for a health savings account (HSA) or be paired with a health reimbursement arrangement (HRA) to help pay for covered health care expenses.
