Community Award Application
Is your community helping citizens lead healthy lives? Do you think your community should be acknowledged for the great things it is doing to make the healthy choice the easy choice for residents? If yes, keep reading to learn how to showcase the amazing work that's been accomplished. Your community could be recognized by Healthiest State Initiative and receive an award and funding to help achieve even more.

The Healthy HometownSM Powered by Wellmark tactics reference guide PDF File shows you a list of the tactics your community can complete to be eligible for the Healthy Hometown Community Award.
Your community will need to have executed at least four tactics between July 1, 2017 and June 30, 2018 to be eligible. Be sure to use the tactic number and letter from the guide when filling out the application, so we can see how your community is helping people eat well, move more and feel better.
Tactics can be newly implemented in the community or they can be existing tactics that have been expanded or enhanced in the community between July 1, 2017 and June 30, 2018.
Healthy Hometown Community Award(s) applications will be accepted until 5 p.m. on Monday, Nov. 5, 2018.
The Healthy Hometown Community Award(s) will be presented Thursday, Feb. 7, 2019 during the 2nd Annual Healthiest State Initiative Awards External Site event at the Ron Pearson Center in West Des Moines, Iowa.
