Mail Service Pharmacy
Walgreens Mail Service is Wellmark’s mail order provider. Not all pharmacy
plans have mail service benefits. Please check your benefits certificate,
coverage manual, or policy to see if you have mail service benefits.
Steps for Using Walgreens Mail Service
- Ask your doctor to write two prescriptions:
- One for a short-term supply (e.g., 30 days) to be filled immediately
at a participating retail pharmacy
- One for the maximum days’ supply allowed by your plan (e.g., 90
days) with as many as three refills (if appropriate) to mail to Walgreens
Mail Service
- Print and complete the Registration &
Prescription Order Form. The registration form includes important
health, allergy, and plan identification information. You can also register
by calling the Walgreens Mail Service customer service team toll-free
at 1-866-611-5961.
- Mail the completed Registration & Prescription Order Form, prescription,
and payment to:

Major credit cards, checks, or money orders are accepted. Do not send
cash.
- Allow 10-14 days from the time Walgreens Mail Service Refills your
order, for delivery of your medication.
You may also ask your doctor to use the Fax
Order Form. Fill out your information on the Fax Order Form and then
ask your doctor to write your prescription on the form. To be valid, the
Fax Order Form must be faxed to Walgreens Mail Service at 1-800-332-9581
from the doctor’s office.
