Pristiq is an antidepressant that was new to the marketplace in May of 2008.

Program Details

For Pristiq to be a covered benefit, you must follow these steps:

You must try and fail BOTH a generic (Tier 1) antidepressant and a preferred (Tier 2) antidepressant before coverage is available for Pristiq.

If your doctor wants you to “skip” steps, your doctor will need to submit a prior authorization request and receive approval for the drug to be covered.
