• Sold with Classic Blue or Alliance Select health plans
• All covered services are applied to the deductible
• Alliance Select must apply the deductible to all services including office visits
• Preventive care can be covered at 100 percent
• Prescription drugs are part of the health coverage and are subject to the deductible; no drug card is necessary
• Minimum $1,000 single deductible
• Maximum single deductible: $2,500 Alliance Select; $5,000 Classic Blue
• Coinsurance only; no copayments
