Voluntary
Group Products
Voluntary Group Products are similar to Basic Group Products in design
and product offerings. However, the employee is responsible for paying
the entire premium. The employer agrees to offer the products and to
deduct the premiums from the employee's paycheck. The choice to buy or
not to buy--and how much--is the employee's decision. Group Voluntary
Products allow employers to offer a complete portfolio of high quality
group insurance products to employees without incurring the cost of the
benefits.
The Voluntary Group Term Life, Accidental Death & Dismemberment, Short
Term Disability, Long Term Disability, and Dental coverages are underwritten
by Fort Dearborn Life Insurance Company.

Term Life
Additional term life insurance at group rates.
- One employee minimum
- Lump sum benefit paid upon death from accident or illness, on or
off the job
- Benefit amounts up to $500,000 in $10,000 increments for employee
or spouse
- Non-tobacco and tobacco user rates
- Children plans available
- Portable
- Convertible to permanent life insurance plans
- Waiver of premium benefit automatically included
- Guaranteed issue available depending on group size and participation
rate

Accidental Death and
Dismemberment (AD&D)
Lump sum benefit paid for a death or dismemberment
due to accident only.
- One-employee minimum
- Benefits from $10,000 to $500,000
- 24-hour protection on or off the job
- Family plans available
- Additional benefit payable for loss of life in a covered accident
while using a seat belt
- Some limitations may apply, including any loss due to suicide or
attempted suicide; see policy for details
Short-Term Disability
Monthly benefits to replace income lost due to disability.
- One employee minimum
- Choice of benefits payable for periods up 52 weeks
- Benefit amounts from $100 to $750 per week; in $50 increments up
to 70% of covered wage
- Non-occupational coverage only
- Partial disability benefit included
- Guaranteed issue
- All industries eligible
- Exclusion period for pre-existing conditions applies
- Some limitations may apply
Long-Term Disability
Monthly benefits to replace income lost due to disability.
- Two employee minimum
- Benefit amounts of 60 percent of salary up to $5,000 per month
- Choice of benefits payable for two or five years, or to age 65;
choices are determined by group size and participation rate
- Benefits begin after 90 or 180 days
- Guaranteed issue
- Partial disability, recurrent disability included
- Other benefits are included
- All industries eligible
- Exclusion period for pre-existing conditions applies

Dental Plans
Comprehensive dental coverage at group rates.
- Two employee minimum
- Two plans are available with two benefit options each
- $1,000 per year benefit maximum per person
- $1,000 per lifetime child orthodontia available on some plans
