• Choose Option 1
• Your call will be answered by the Catalyst Rx Center of Excellence staff in Des Moines
• Most prior authorizations will be completed in less than 10 minutes
• Prior authorization support is available 24 hours a day, seven days a week

Request Prior Authorization Via Fax

• Download the appropriate prior authorization form from the Iowa provider forms section or South Dakota provider forms section; depending upon the drug, there is a separate form for each drug or each class of drugs
• The forms are interactive, which means you can complete the forms online and then print the forms for faxing
• Fax the completed form to 1-866-884-4345
• An approval or denial will be faxed back to you by the next business day
