Enrollment of Facilities/Entities in South Dakota

Facilities and Entities can participate with Wellmark by one of two contracting mechanisms. The terms Contracts and Agreements may be used interchangeably.

1.Contract through a Provider Organization (PO) or a Provider Hospital Organization (PHO). Typically if a provider contracts through a PO or PHO, this organization “owns” the contract with Wellmark. The organization is accountable to facilitate the contracting portion of the enrollment process for their members.

2. Providers may contract directly with Wellmark as individuals and, as such, the individual provider “ owns” the contract with Wellmark.

If a hospital owns and separately bills additional services such as a home medical equipment supplies or home infusion therapy, the hospital must sign entity agreements for these services because reimbursement information for these provider types is not included in the hospital agreement.
