How the Process Works: Obtain a Provider Number

In order to submit claims to Wellmark, eligible providers must obtain a Wellmark-assigned provider number. Refer to the following table to begin the process:

If ... Then ...
You are an eligible provider seeking Wellmark network participation Complete one of the following applications:
  • Iowa Statewide Universal Practitioner Application 
  • Professional Provider Application
You are an eligible provider who does not wish to participate with Wellmark Complete the Professional Provider Application to obtain a provider number to submit claims.
You have completed the Iowa Statewide Universal Practitioner Application for another payer, managed care organization (MCO), or other entity Complete the Wellmark Addendum included as part of the Iowa Statewide Universal Practitioner Application.

General Rules

General rules regarding provider number assignment:

  • All providers are assigned a unique provider number.
  • Clinics that use a common tax ID number may also obtain a unique clinic billing number. Providers associated with that clinic (tax ID #) are identified through individual non-billing numbers tied to the unique clinic billing number.
  • All primary care providers (PCPs) and OB-GYN providers applying to participate in Blue AccessSM, Blue Choice®, and Blue Advantage®, must have a unique provider number for each site from which they provide services. This assists Customer Service staff to identify the correct practice location when answering inquiries and allows Membership staff to assign HMO members to the appropriate site. It also facilitates the site review process required for Wellmark's credentialing process.
  • Specialty and allied providers may choose to have a unique individual provider number at each practice site. However, providers who have locations affiliated with different tax ID numbers or billing addresses must have a unique individual provider number for those locations.
