Credentialing: Site Visit

As part of the credentialing process for all primary care providers (PCPs), OB/GYNs, and psychiatrists, a Wellmark Provider Relations Manager will contact your office to set up a time for an office site visit. Once scheduled, the Provider Relations Manager will:

• Fax and mail to your office manager a listing of the site review criteria
• Provide his/her name and phone number for questions

At a minimum, the review must include all site visit indicators and measures established by the Credentialing Committee and the review of one medical record to ensure the office has a systematic method of recordkeeping.

Once this process is completed sucessfully, your profile is presented to the Credentialing Committee.
