How the Process Works: Obtain a Provider Number

Eligible providers must obtain a Wellmark-assigned provider number prior to submitting claims to Wellmark. Provider numbers are typically assigned as part of the application and contracting process. Providers who do not wish to participate with Wellmark must obtain a provider number to submit claims.

To obtain a provider number, complete the following application:

• Iowa Statewide Universal Facility Application

General Rules

General rules regarding provider number assignment:

• Wellmark will use the number assigned by Centers for Medicare and Medicaid Services (CMS)
• If CMS doesn't assign a number, Wellmark will assign a unique provider number
