Requirements for Taxpayer Identification and Social Security Numbers

When you submit your National Provider Identifier (NPI) to Wellmark Blue Cross and Blue Shield, Wellmark updates its provider files and maps the information to your existing Wellmark provider number.

Wellmark's provider files contain other numbers you previously reported, such as your taxpayer identification number (TIN) and/or social security number (SSN). If you are submitting claims with a different TIN and/or SSN than is on the Wellmark provider files, your claims may reject. It’s important that the TIN and/or SSN you are using is consistent across the carriers to ensure that claims continue to process accurately.

Please provide your current information when you send Wellmark your NPI. Submit all NPIs and corresponding numbers that belong to your organization.

If you have already submitted your NPI and need to change the TIN Wellmark has on file, complete the Provider Change Form and fax a completed W-9 Form (PDF) to 515-245-4884.

To verify that Wellmark has an accurate TIN or SSN on file, contact our Network Administration department at providercredentialing@wellmark.com or 800-708-1342, Option 2.
