Reporting Your NPI to Wellmark

If you have never enrolled with Wellmark, you must complete the Iowa or South Dakota enrollment process to submit claims using their National Provider Identifier (NPI). Wellmark uses information received during the enrollment process to create a record that enables us to process claims. The process needs to be completed before an NPI can be used as the billing number or a rendering number on a claim.

Wellmark returns enrollment applications that do not include your National Provider Identifier (NPI).

Examples of provider types who might not previously have enrolled include:

  • Advanced registered nurse practitioners
  • Physician assistants
  • Social workers
  • Audiologists

Our Iowa and South Dakota enrollment pages include a list of provider types who are eligible to enroll.

We have received NPIs for those providers who are active in our systems. If you have changes to your NPI, please report the change using Wellmark’s NPI Reporting Form. Be sure to select the “Change to Existing Notification” option.
