Living a healthy life can be complicated when you’re managing a long-term
condition. But having someone assist you along the way can make a difference.
The Disease Management program provides you with personalized nurse
support, educational materials, and other support services.
You’ll be invited to participate in
the program if you’re diagnosed with:
- Asthma
- Cardiac: Heart failure or coronary artery disease
- Chronic Obstructive Pulmonary Disease (COPD)
- Diabetes
Program participation is voluntary and provided at
no additional cost to you.
Your program nurse provides ongoing support and reinforces your physician’s
treatment plan by:*
- Talking with you about your condition(s)
- Reviewing your medications and current
treatment
- Assisting you in goal setting and creating an action plan
- Helping you understand
your doctor’s recommendations
- Connecting you to other programs, as needed
