Health Dimensions brings together a continuum of health management
solutions to help improve your customers’ overall health, prevent
illness, and optimize their health care dollars.
This comprehensive suite of health and wellness programs touches
people in every stage of health. Whole Health Dimensions:
- Helps those in good health stay healthy and prevent disease.
- Offers one-on-one support for people dealing with chronic conditions,
or severe and complex illnesses.
- Provides for member's health needs to be integrated throughout
the entire continuum of services to ensure coordination of care.
Fully insured members: Whole Health Dimensions
programs and services are integrated into the member's Wellmark
health plan at no additional cost. This includes both members who
are part of a group, as well as individual members under age 65.
Members of self-funded groups: Self-funded groups
may purchase the Whole Health Dimensions programs for their employees.
Programs & Services
Whole Health Dimensions includes the following programs and services:
- 24/7 Personal Health Assistant. One call connects members
to around-the-clock assistance from specially trained health professionals:
- Nurse Support 24/7: Advice for urgent health care concerns.
- Care Navigation 24/7: Help in locating health care resources
and understanding specific medical treatments.
- Disease Management. Provides personalized nurse support,
educational materials, and other support services for members
who have been diagnosed with the following conditions:
- Asthma
- Heart failure
- Coronary artery disease
- Chronic Obstructive Pulmonary Disease (COPD)
- Diabetes
- Pregnancy Care Program. Provides resources to help
all expecting mothers better understand and manage their pregnancy.
The program proactively identifies women at risk for prenatal
health complications, so an obstetrical nurse can work with the
member and her health care team to help the pregnancy be as healthy
as possible.
- Health & Wellness Tools. Web-based tools on www.wellmark.com
that offer personalized information to help members make healthy
decisions. A Personal Health Assessment (PHA) helps members evaluate
their health status and risk levels, and take action to improve
their health.
- Pharmacy Management. Prescription drug benefit programs
to ensure clinical efficacy and drive down pharmacy health care
costs.
- Utilization Management. Manages costs by evaluating
the medical need and efficiency of health care services, procedures,
and facilities.
- Complex Case Management. One-on-one support to maximize
benefits for members facing a serious illness, surgery, or injury.
