Charitable Contributions
It is our belief that the most important contribution we can make is to
help improve the health of Iowans and South Dakotans.
Within the scope of our charitable contributions program, qualifying
non-profit and health-related organizations may apply for and receive
financial support from us for specific programs and events that may have a
positive impact on health issues facing Iowans and South Dakotans.
Wellmark also allocates funds to civic and community projects and programs
that preserve and enhance the quality of life within the communities we
serve.
All requests for charitable contributions are considered on a quarterly
basis by the Wellmark Corporate Contributions Committee.
Contribution requests for a specific event must be received a minimum of
120 days prior to the event date.

Limitations |

Generally corporate charitable contributions will not exceed $5,000. |

Wellmark Blue Cross and Blue Shield does not continuously fund any
organization or event. |
| Wellmark Blue Cross and Blue Shield generally does not make
multi-year contributions. |
Contributions Exclusions
Wellmark Blue Cross and Blue Shield will not generally consider
requests for charitable contributions for or from:

| United Way organizations seeking funds for United Way-funded

Individuals |
| Capital campaigns |

Endowments |
| Political campaigns, candidates, parties or partisan

Sectarian, denominational, or religious organizations for support of
theological functions |
| Scholarships, fellowships or internships |
|
Athletes or athletic events |
| Private foundations |

Social organizations |
| Labor or fraternal groups, unless contribution will be used
to address a documented community health-related need |

Fund-raising events if more that 20 percent of funds raised are
