• Higher coinsurance for most covered services to keep your premiums low – pay more for health care services only when you need them
• Prescription drug coverage includes generics only, with limited exceptions
• Coverage for mental health and chemical dependency treatment, with payment and benefit limitations

• Contraceptive — Covers generic and brand name oral contraceptives and contraceptive devices that are used to prevent pregnancy
• Dental — Covers diagnostic and preventive care, and major and minor restorative care
• Life Insurance — Low-cost insurance available through USAble Life Insurance Company. Benefit amounts start at $10,000, and premium rates are determined by age and gender
• Vision — 10% to 60% discount on eyeglasses, contact lenses, non-prescription sunglasses, and other vision items. Discounts on eye exams available in many areas. Offered through Avesis, which contracts with more than 10,000 retail optical outlets nationwide
