When you apply for Alliance Select PPO Comprehensive, you will choose from among these optional benefits that can be provided for an additional premium:
- Contraceptive — Covers oral contraceptives and contraceptive devices that are used to prevent pregnancy
- Dental — Covers diagnostic and preventive care, and major and minor restorative care
- Supplemental Accident — Supplements your regular insurance in case of an accident
- Life Insurance — Low-cost insurance available through USAble Life Insurance Company. Benefit amounts start at $10,000, and premium rates are determined by age and gender
- Vision — 10% to 60% discount on eyeglasses, contact lenses, non-prescription sunglasses, and other vision items. Discounts on eye exams available in many areas. Offered through Avesis, which contracts with more than 10,000 retail optical outlets nationwide
