One Call Connect Example: Prescription Non-Compliance

Patient B has multiple, complex medical conditions, and isn’t adhering to your medication plan or treatment regimen. Your office nurse gives the member’s contact information to One Call Connect, then:

  1. One Call Connect passes the information to a Whole Health Dimensions disease or case management nurse.
  2. The program nurse engages the patient on your behalf, learns about his challenges, and uses behavioral change techniques to help the member set goals.
  3. Your office receives an update on the program nurse’s activities.
