Physical Medicine Pilot on Quality

The Wellmark Blue Cross and Blue Shield Collaboration on Quality^® (CoQ) Physical Medicine Pilot on Quality, developed in collaboration with physical medicine providers, is a voluntary initiative. A total of 238 Iowa and South Dakota chiropractors, physical therapists, and occupational therapists participated in the 2008, submitting information on care provided to 5500 Wellmark members with musculoskeletal disorders.

How Does the Program Work?

• Participants provide data on the care they provide Wellmark members by submitting physical medicine Care Plans electronically.
• Data is evaluated and compared to recognized best practice standards.
• Program participants are identified in the Wellmark online provider directory.
• Providers receive resources and services that support and promote process improvement. 

What Forms Are Used?

• Initial Care Plan - #S-2306  (216KB)- Use the Initial Care Plan after a member’s first visit.
• Extension of Care Plan - #S-2307  (187KB) - Use the Extension of Care Plan to request additional services or visits beyond what was outlined in the Initial Care Plan, or to report a change in a patient's condition or diagnosis.
• CoQ^® Physical Medicine Discharge/Outcomes of Care - #S-2313  (138KB) - Use the Discharge Outcome Summary form to report final diagnosis if different from initial diagnosis, patient overall improvement, referral to other services.
