The following medical policies were removed from our medical policy pages:
- Bariatric Surgery for Morbid Obesity
- Rhinoplasty or Septorhinoplasty
The peer-reviewed literature and clinical practice guidelines for these medical services have built to a consensus around a standard of care. Thus, Wellmark will remove these medical policies and apply InterQual® Criteria – for determining the medical necessity of pre-service inquiries.
Prior approval continues to be recommended for these services. Go to the Prior Approval page to view the list of services for which prior approval is recommended. Go to the Forms page to access the pre-service inquiry form.
It is also recommended to check the member’s benefits to see if the service is covered.
