Effective October 1, 2011, Hy-Vee Pharmacy Solutions (HPS) will replace Fairview Specialty as one of our two preferred specialty pharmacy providers. If a member's benefits have previously required the use of either Fairview or Caremark as their specialty pharmacy provider, they will be required to use HPS or Caremark Specialty Pharmacy.

 

Both Caremark and HPS have been exclusively chosen due to their excellent customer service, professional expertise, and competitive rates.  Members will have improved access to specialty drugs with this larger network. With the addition of HPS, members also have the option of having their specialty drug delivered to their local Hy-Vee pharmacy or their home.

 

Members have been sent a letter notifying them of the change in specialty pharmacy providers. Along with the letter, members were sent a short enrollment form they will need to complete to take advantage of the services offered through Hy-Vee Pharmacy Solutions.
