Medical Policies for Varicose Vein Treatments

In response to physicians’ requests for clearer medical necessity criteria for the treatment of varicose veins, Wellmark has reviewed, updated, and divided the treatment of varicose veins information into three separate medical policies:

1. Conventional Treatments of Varicose Veins
2. Endoluminal Radiofrequency or Laser Ablation of Varicose Veins 
3. Sclerotherapy of Varicose Veins
