Eligibility for Group Coverage

Answer the following questions to learn whether your company is eligible for group insurance.

  • Are you an Iowa-based business? 
    Your business must have a home office in the state of Iowa.
  • How many employees do you have on your payroll? 
    You must have a minimum of two full-time employees who work at least 30 hours weekly.
  • Do you have the documentation required to determine group eligibility? 

    You must have a Federal Tax ID number or Sole Proprietor documentation. Additional documentation needed may include:

    • Employer’s Quarterly Contribution and Payroll Report for the most recent quarter
    • Wage and Tax Statement
    • W-2s/W-4s for employees
    • Schedule K-1
  • Will you be paying any portion of the premium? 
    We recommend you contribute at least 75% of the premiums for your employees; you must contribute at least 25% of the premiums for the individual employee if you are an employer with 2 - 50 employees and 50% if you are an employer with 51 - 100 employees. 
  • How many employees want to participate in your health plan?
    We require 75% of all eligible employees who do not have existing health coverage to participate in your company's Wellmark coverage.

If you meet all of these requirements, your company is eligible for a Wellmark small business health plan. Request a quote.
