Submit Claims Use the online flex claim system to submit your flexible spending account claims:

• Go to www.myflexonline.com
• If you haven’t registered yet, select New User and complete the form to create an account.If you're already registered, log in with your user name and password and select Request Payment.
• Select the claim type from the drop-down menu.
• Enter the requested information about the claim.
• Print your completed form by selecting View Form and Continue. Sign and date the form, attach your receipts, and send to Wellmark using one of these methods:
  • Mail: P.O. Box 14585, Des Moines, IA 50306-3585
  • Fax: (515) 248-5350
  • E-mail: wellfsa@wellmark.com (scan your claim form and receipts to e-mail)

Debit Card Claims
If you use the Blue Priority Flex^SM Debit Card:

• Please wait until you receive a flex debit card receipt request letter before providing documentation for flex debit card claims.
• Receipt request letters are mailed the month following the transaction month.

Orthodontia Claims

• Submit copies of receipts for payment/services that have incurred for current plan year, or
• Complete the Orthodontic Payment Form  (215KB) to receive recurring monthly payments
