How to find your plan
You can find the name of your plan in a variety of ways:
  • Look on the top right corner of your member identification (ID) card.
  • Log in to myWellmark (My Benefits > Plan Details).
  • Call the toll-free number found on your ID card.
Then use your specific plan to locate a participating doctor or hospital. If you have not yet enrolled in a plan, please check with your employer or broker to see which networks will be available.
Why is it important to choose a plan?
Using providers that are in your specific plan (choosing "in-network" care) means you'll pay less out of your own pocket for care, saving you money. An in-network provider is one who is contracted with Wellmark to provide services to plan members for specific pre-negotiated rates.

Plan descriptions

Alliance Select
  • Alliance Select is a preferred provider organization (PPO) available through Wellmark Blue Cross and Blue Shield of Iowa.
  • You can see any provider you choose, however, if you see a provider who participates in the Alliance Select network, you will have lower out-of-pocket expenses, and claims will be filed for you.
  • If you have Alliance Select coverage, but are seeking services in South Dakota, please refer to the Blue Select® directory to locate a participating provider in South Dakota.

Blue Access®
  • Benefits are available from any provider or facility that participates in the Wellmark Health Plan of Iowa, Inc. network.
  • Generally, there are no benefits for services received outside of the Wellmark Health Plan of Iowa network, except for emergencies or accidental injuries.
Blue Advantage®
  • Blue Advantage requires you to designate a primary care provider (PCP) from a list of doctors credentialed by Wellmark Health Plan of Iowa, Inc. The primary care provider you select will provide or coordinate your care.
  • Generally, there are no benefits for services received outside of the Wellmark Health Plan of Iowa network, except for emergencies or accidental injuries.
Blue Choice®
  • Blue Choice requires you to designate a primary care provider (PCP) from a list of doctors credentialed by Wellmark Health Plan of Iowa, Inc.
  • There are three levels of benefits.
    • Level 1 benefits - when your care is provided or coordinated by your designated PCP.
    • Level 2 benefits - when you visit any Blue Choice provider and your PCP does not coordinate your care.
    • Level 3 benefits - when you receive care from a provider who is not in the Blue Choice network. Your out-of-pocket costs are the highest with Level 3 benefits.
Blue Select®
  • Blue Select is a preferred provider organization (PPO) available through Wellmark Blue Cross and Blue Shield of South Dakota.
  • With a preferred provider organization, you can see any provider you choose, however, if you see a provider who participates in the Blue Select network, you will have lower out-of-pocket expenses, and claims will be filed for you.
  • If you have Blue Select coverage, but are seeking services in Iowa, please refer to the Alliance Select directory to locate a participating provider in Iowa.

Classic Blue® (Iowa)
  • With Classic Blue, you can visit any provider for your medical care, but you will receive the benefit of negotiated pricing and electronic claims filing when care is coordinated through providers who participate in the Classic Blue network.
  • If you have Iowa Classic Blue coverage, but are seeking services in South Dakota, please refer to the Classic Blue South Dakota directory to locate a participating provider in South Dakota.
Classic Blue® (South Dakota)
  • With Classic Blue, you can visit any provider for your medical care, but you will receive the benefit of negotiated pricing and electronic claims filing when care is coordinated through providers who participate in the Classic Blue network.
  • If you have South Dakota Classic Blue coverage, but are seeking services in Iowa, please refer to the Classic Blue Iowa directory to locate a participating provider in Iowa.
hawk-i Blue Access®
  • hawk-i (Healthy and Well Kids in Iowa) enrollees have Blue Access network coverage.
  • hawk-i Blue Access is available through Wellmark Health Plan of Iowa, Inc.
  • Benefits are available whenever covered services are received from a hawk-i Blue Access network provider. Generally, there are no benefits for services received outside of the network, except for emergencies or accidental injuries.
Group Sponsored Networks
Iowa Health System Employer Sponsored Network
  • A network of providers has been developed to support the Iowa Health System benefit plans. This network will only be applicable to Iowa Health System employees.
