^{*Prevacid OTC (over-the-counter) is now available without a prescription at your local pharmacy.  The over-the-counter version of Prevacid is available and labeled for short-term or intermittent use in a 15MG capsule.} 

^{**It is anticipated in mid-December 2009, a generic equivalent of Valtrex will be available.  The generic name is valacyclovir. Once the generic is readily available, Valtrex will move to Tier 3.}
