How to Participate and Frequently Asked Questions

Find answers below to frequently asked questions about the Personal Health Assessment and obtaining health screening results (biometrics) from your physician.

 

Please note: In order to receive your participation incentive, you must complete the Personal Health Assessment, including biometric information (lab results) by December 4.

Lab Results (Biometric Information)

What do I request when I call my physician for my lab work?
Tell the scheduler that you would like a routine physical, including a blood draw, and that this is part of your routine preventive care benefits. Be sure to have the Your First Steps to Wellness pdf (1.2MB) guide available to help ensure you receive the information needed to complete the PHA.

 

What if I had a routine physical recently?

You can enter your most recent lab results into the PHA. If you have had a physical within the last year and do not have your results, request a copy of your lab results from your physician. Be sure to have the Your First Steps to Wellness pdf (1.2MB) guide available to help ensure you receive the information needed to complete the PHA.

 

I do not have a primary care physician. How do I find one?
Call Wellmark Customer Service at the number listed on the back of your Wellmark ID card or visit Find a Doctor or Hospital.

Personal Health Assessment

About the PHA

What is a Personal Health Assessment (PHA)?
A PHA is a questionnaire that collects information about your health habits (i.e. physical activity, nutrition, stress management), biometric information (i.e. cholesterol, glucose, blood pressure, height and weight), and assesses your readiness to make healthy changes. The PHA can be completed online (paper available on request) and takes about 20 minutes to complete.

 

When will the PHA be available?
Casey's health plan members, including spouses, may access the PHA between November 9th and December 4th. Please remember that you need to obtain your lab results prior to completing the PHA. Follow the Your First Steps to Wellness pdf (1.2MB) guide for step-by-step instructions. Remember, to be eligible for the incentive, you must complete the PHA (including lab results) by December 4th.

 

How does completing a PHA benefit me?
A PHA effectively identifies risk factors that may not be detected with a health screening alone. Ultimately, this program empowers you to improve your health by helping you better understand your risk factors and provides information and tools to help you make better health-related decisions. In addition, as individuals achieve and maintain good health, we can better manage health care costs for you and for the health plan.

 

Once you submit your PHA, you will immediately receive a comprehensive health status report which helps you learn more about your personal health risks and opportunities for health improvement. You will also receive recommendations for programs which can support you as you work to make healthy changes.

 

Who has access to my personal information?
The PHA is administered by Wellmark Blue Cross and Blue Shield. It is not administered by Casey's. Your information will be used in the following ways:

  • Your personal health status report is available for your use as a guide to improving your health. Your personal health information will not be shared with Casey's.
  • Casey's does receive a list of participants' names which enables a Human Resources staff member to award the participation incentive ($25 gift card for online PHA and $10 for paper PHA). They also receive non-individualized (percentages and totals) group health data which gives the wellness committee a picture of the results for the entire group. They will use this information to help them plan initiatives to address the health needs of the workforce.
  • Your health plan administrator, Wellmark Blue Cross and Blue Shield, accesses pertinent health information which helps the Wellmark Health Support Team identify services, such as chronic condition management and telephonic health coaching, which can assist you in achieving your health-related goals. A health professional from Wellmark may reach out to you to offer support as you work to improve your health.

Completing the PHA

What information do I need to have ready before I access my PHA?

Reference the Your First Steps to Wellness pdf (1.2MB) guide (sent to health plan members' homes) as you walk through completing the PHA and enter your biometric data.

 

What if I do not have access to a computer to take my PHA?
A paper version of the PHA in English or Spanish may be obtained by contacting the Wellness Help Desk at 1-877-252-8412. Please note that once a paper PHA is submitted, it may take up to four weeks to receive your personal health status report.

 

Who do I contact for help with completing the PHA?
Questions about registering for and logging into www.wellmark.com may be directed to the Wellmark Technical Help Desk at 1-800-407-0267 (open Monday - Friday, 7 a.m. - 5:30 p.m. CST). Please direct questions regarding PHA completion to the Wellness Help Desk at 1-877-252-8412 (open Monday - Friday, 8:30 a.m. - 8:30 p.m., Saturday, 9 a.m. - 5:30 p.m. CST).

 

How do I receive the incentive?
Every employee or spouse on the health plan who submits a PHA (including lab results) by December 4th will receive a Casey's gift card ($25 for online and $10 for paper) via mail within a few weeks of completion.

 

Is the PHA open to spouses?
Yes. Spouses covered by Casey's Wellmark Blue Cross and Blue Shield health plan are encouraged to complete the PHA. Each individual will need a separate e-mail address to complete the PHA and to receive their PHA report. Please share the Your First Steps to Wellness pdf (1.2MB) guide and make copies of the Biometric Patient Result Sheet if necessary.

 

What if I do not have my own e-mail address as is required to access the PHA?
You can create a free e-mail account through sources such as www.hotmail.com Leave site or www.gmail.com Leave site. Please note: Each individual will need a separate e-mail address to complete the PHA and to receive their PHA report.

Taking Action to Improve Your Health

How can I best use the information I receive from the PHA to improve my health?
Share the results with your physician, so he or she can work with you to identify areas of concern and offer recommendations for improving your health. In addition, if you have questions about your PHA, your lab results, or other health concerns, Personal Health Assistant 24/7 can help you with the answers. When you call 1-800-724-9122, a specially trained health professional can help you understand what your lab results mean.

 

What programs are available to help me improve my health?
Casey's health plan members have the following services available to them at no cost through the health plan administrator, Wellmark Blue Cross and Blue Shield (some services are based on health risk and may not be available to all members):

  • Online Lifestyle Management Programs — Web-based tools designed to help you improve your health, including interactive programs related to stress management, depression management, weight management, smoking cessation, and medication adherence.
  • Telephonic Health Coaching — Available for employees who are at higher risk for chronic disease, this program provides one-on-one telephonic counseling to help participants improve their health.
  • Chronic Condition Management — One-on-one telephonic support available for members with a chronic condition, including asthma, chronic obstructive pulmonary disease (COPD), congestive heart failure, coronary artery disease, and diabetes. Support is also available for individuals who have been diagnosed with certain types of cancer.
