Healthy Directions Employee Wellness Program
Welcome to Casey's new Healthy Directions Employee Wellness program! Our goal is to provide tools and resources to help you understand your health today and the steps you can take to live a healthier tomorrow.
Casey's invites health plan members, including covered spouses, to take a Personal Health Assessment (PHA). A PHA is a confidential questionnaire that collects information about health habits, health history, biometric information (i.e. cholesterol and blood pressure), and assesses readiness to make healthy changes. After completing the PHA, you will receive a personalized report that will help you understand your results and the opportunities you have to improve your health.
Completing the Personal Health Assessment is as easy as 1, 2, 3!
Step 1: Obtain your lab results.
Step 2: Complete the PHA  , including lab results, between November 9 and December 4. Within a few weeks of completing the PHA, you will receive a $25 Casey's gift card for completing your PHA online and a $10 Casey's gift card for completing the PHA via paper.
Step 3: Review your personal health report and create a plan for improving your health using the tools available to you through the health plan.
